  2. ሳይቱ በመልሶ ማልማት አካባቢ ውስጥ የሚገኝ ከሆነ የግንባታ ሥራው ሊጀመር እንደሚችል የሚያሳይ ከሚመለከተው አካል የተሰጠ ስምምነት መቅረብ አለበት፡
  3. የግንባታ ፈቃድ ቅጽ ሞልተው ያቅርቡ
  4. የፕላን ማሻሻያ ለመጠየቅ የፕላን ማሻሻያ መጠየቂያ ቅጽ ሞልተው ያቅርቡ
  5. የድርጅቱ ማኅተም የተያያዘበት የአማካሪው ግዴታ ማረጋገጫ ቅጽ
  6. በአቅራቢያው ከሚገኘው አዋሳኝ በ2 ሜትር ርቀት ላይ ለሚካሄድ የግንባታ ሥራ የአዋሳኝ ግንባታ መግለጫ ቅጽ
  7. የአማካሪው ድርጅት ማህተም የተያያዘበት የጠቅላላ ወጪ ቅጽ
